CASE MANAGER I – GROUP HOME
This part-time (16 hrs/wk) night shift position is for our 24-hour Group Home located in Indianapolis.
Duties include coaching seriously and persistently mentally ill adults to be independent through daily
structured activities (meal preparation, housekeeping tasks, etc.), transporting residents, monitoring
their health and medication, etc. Requires a high school diploma/equivalent with 1 year of experience
preferred.
Eligible for shift premium.

CASE MANAGER II – CLUSTER APARTMENTS
These positions are for our semi-independent living program located in Indianapolis. Duties include teaching
daily living skills to seriously and persistently mentally ill adults along with case coordination, linking
clients to benefits, support of treatment plan, medication monitoring/management, etc. Requires a Bachelor’s
degree in psychology-related field with 1 year of related experience preferred.
• Part-Time (20hrs/wk/Days/Evenings) – Lakeside Pointe
• Full-Time (40hrs/wk/Evenings) – Lakeside Pointe

This On-Call position (any/all shifts, no set schedule, no guarantee of hours, PRN) is for our 24-hour Deaf Group
Home located in Indianapolis. Duties include coaching seriously and persistently mentally ill deaf adults to
be independent through daily structured activities (meal preparation, housekeeping tasks, etc.), transporting
residents, monitoring resident’s health and medication, etc. Requires a high school diploma/equivalent with
1 year of related experience preferred. Fluency in ASL and adaptive equipment is required.
