When you come
for your first appointment, please bring any of the following documents that apply to you
and will assist us in verifying your income. These may include one or more of the following:

 

• Copy of your latest U.S. Individual Income Tax Return

• Copy of your last three (3) pay check stubs

• Letter from Social Security verifying your income.

• If you are unemployed, a verification of unemployment can be obtained from your nearest unemployment office.

• Copy of child support checks

• Court and/or legal documents that will verify your income

• Assistance for Families with Dependent Children (AFDC)

• Assistance for Families with Dependent Children (TANF)

• Food Stamps

 

 

 

 

CHARGES FOR SERVICES

There are set charges for all outpatient services provided at BehaviorCorp.

At the time of your first appointment we will conduct a financial interview with you to determine if you have private insurance, Medicare or Medicaid.

If you have private insurance, Medicare or Medicaid, you will be responsible for 100% of your deductible, co-payment, coinsurance or spenddown. There may also be additional charges that Medicare, Medicaid or your private insurance identifies as your responsibility.

If you do not have insurance, you will be responsible to make full payment for the services you receive. If you qualify, a percentage of the total charges will be set. That percentage can range from 100% to as low as 10%.
